When consecutive FAIL has been committed repeatedly by said FAIL offender, you finally must call FAILNANEGANS and interfere so that another FAIL can possibly be prevented.

Failnale 

An unsatisfying finale. Riddled with plot-holes, dei ex machina (plural of deus ex machina), and various other writing blunders. The name is a portmanteau from the words "fail" and "finale".
Normally stemming from either a premature cancellation, or writers writing their way into a corner.
What makes a finale a "failnale" is completely subjective, and is a decision up to each person to make. However, there are times when the writing is so blatantly bad, only the most hardcore of fans would defend it.

Note : While it mainly refers to television programs it is also applicable to, books, video games, comics, and basically any form of entertainment that eventually ends.
Rachel : "So during the finale there was an ancient and powerful being, who just happens to show up to give the main guy a brand new power RIGHT when he needed it."
Steven : "That was total garbage! They didn't even foreshadow something as big as that. It just showed up out of nowhere! And did you see what they did to the villain?"
Rachel : "Are you talking about the convenient and sudden mental breakdown? It was only there so the good guys could easily win. What a cop-out"

1.) The process of facepalm, but a step beyond. You instead, go for the facepalm, but miss the face.

2.) This is a higher degree than a facepalm, when facepalm just isn't good enough.
Person: Hey, guys!
You: What?

A grenade, often in FPS games, that does not go where intended that may:

1) Bounce back and harm and/or kill you.

2) Miss the target completely.

failalyzer 

The use of a breathalyzers electronic results on a printed sheet as admissible evidence in a court of law for an OUI (operating under the influence) case, even though the defendant's counsel hasn't been allowed to determine whether or not the source code of the device is faulty.

Any breathalyzer used in court without testimony of the administering officer or without the papers from the manufacturer of the breathalyzer certifying the source code isn't broken or buggy and provides sound results is automatically deemed a failalyzer
Defendant and his Counsel meet before an OUI arraignment:

Counsel: Looks like we're not going to be able to take this to trial since a breathalyzers results are considered 100% accurate

Defendant: Yeah, but I only had one drink, and I wasn't even swerving very much. Are you sure those blood alcohol level results are accurate?

Counsel: Now that I think about it, the only guarantee I have is the manufacturers good will. I don't have any certifications as to the accuracy of the device or soundness of its source code and since we are in a court of a law, we must depend on legal documents; not simply the good will a business offers to its good customers.

Defendant: Yeah, we don't want a failalyzer in court

Counsel: We certainly don't

An event during which everyone goes into a frenzied panic because the government issues a Tsunami Warning and evacuates everyone after an earthquake halfway around the world while the media plays it up like its the coming apocalpyse, only to feel like idiots when nothing actually happens.
did you hear about Hawaii's failnami of 2010? everybody freaked out, bought out the grocery stores and gas stations, evacuated, and nothing even happened. nice one, civil defense.
