A poorly prepared steak, cooked at the incorrect temperature or badly seasoned
A steak cooked well-done in actuality is a big misteak, while a steak cooked medium rare is rarely a misteak.

However, dropping a steak on the ground prior to (or after) placing it on the grill is also a misteak.
