Zueco 

Zueco is a rare condition caused by poor personal hygiene. It’s symptoms are an infestation of barnacle like molluscs in all the bodily crevices, especially the groin and armpits. it is highly contagious. At night or when the carrier is still these molluscs open their shells, extend their palps and grope around for stray organic material to feed on, making a distinctive rustling sound.

I reckon that guy has zueco. You can hear them feeding when it’s quiet.
