Exactly like "Excuse me?", but stronger.
If typed, hyphens can also be used if one wishes, becoming "Ex-fucking-cuse me?".

At a competition, the prize for the first place looks inexplicably more insignificant than that for the second prize.
"Exfuckingcuse me?", goes the winning athlete in his head.

You're walking around the street, when somebody bumps into you all of sudden and drifts off just as quickly. Later on, you meet the same person and - lo and behold - they bump into you again. That's one situation when you may just go and yell: "Exfuckingcuse me? Are you serious?"
