This question is asked to confirm that the inquired person is in a sound physical, mental, or emotional state.

The question "are you okay?" is usually met with a lie, because the person will feel that they don't need to worry the others, or that they just can't explain it, or talk about anything right now.

"Why would I be?"

Hearing this phrase is more important than anything else, but it is not worth pushing if they do not want to say anything. Maybe they are feeling not okay and do not want to talk about it or they are actually fine, no matter what it is better to at least let someone know you are noticing their tendencies and how they are not seeming fine.
