Dump Truck Fire 

Imagine a dump truck on fire. The garbage in the truck is, whether by accident or on purpose, on fire. The fire can be put out but not until the driver stops. Now imagine they won't stop the truck. In use, it describes a failure in leadership to address a serious problem within an organization, because to do so would mean haulting usual operation.
"The fact that our factory keeps producing vibrators with standard hardware, despite the blue prints calling for metric, is a real Dump Truck Fire!"

In entertainment or sports, a laughably poor performance usually caused by:
1. Lack of planning, preparation or talent.
2. Random events that effectively sabotage
the effort (i.e., technical problems).
