In Texas Hold'Em, it is commonly referred to when holding a King and an Ace. It is called such because it is a sweet, sweet hand, but can cause a bit of a toothache.
"Whoa Burt, I just ran my Big Sugar into pocket Aces! I was sure I had the best hand!"
