Scots word but more specifically Glaswegian which is a combination of twat (female genitalia) and tadger (from the more common English term todger referring to a penis). Other forms exist in common Scots parlance e.g. fannnybaws. Quite ahead of its time in being a gender neutral insult.
Used in a derogatory sense, e.g. he’s such a twadger so he is.
