After the main email server crashed, disrupting service across the enterprise, there was a hotwash involving the entire IT department.

A term used in the fire department for a discussion after a fire where out of touch deputy chiefs and battalion chiefs question the decisions of company officers. Generally all feed back is ignored and regardless of outcome all company’s are told they suck.
