It uses as a reply to a dumb/imaginary wish.

One may add it his own 'punch' bolting his uniqueness & individuality and also making the impact much bigger.

Example 1:
David: "Oh simon, wouldnt it be great if we were able to fly?"
Simon: "Yeah...If my grandma had wheels..."
