A blanket statement used to describe something/someone which is severely below par, it can be used in many contexts, particularly common when describing the attractiveness of a person, or an unconventional idea.
Fabio: what do you think of this outfit?
Wilson: it’s miles off it

Dre: I think this girl is well fit, what do you reckon?
Wilson: nah, she’s miles off it

Shane: what do you think of pineapple on pizza?
Wilson: that’s miles off it
