Legendary Japanese Puroresu (professional wrestling) star in Japan who died in the ring in 2009 after taking a backdrop suplex in a tag team match. He died of a cracked spine. Misawa has the destinction of having more official five star matches from The Wrestling Observer than any wrestler in history (over 20). Misawa also was the second incarnation of the legendary Tiger Mask. He took the gimmick from Satoru Sayama. Usually something that isn't a particularly a big deal, esp. when it primarily affects someone else - sort of a mild schadenfreude. Sometimes doubled or tripled, and possibly said in a sing-songy voice for added comedic effect to imitate a muted trumpet, which is how the meme originated.
