This is a term used by heroin addicts (& occasionally others) to describe themselves or other heroin addicts. It may be more prevalent in areas where black tar heroin is the primary variety available.

Man: "I have been searching for hours for keys."
Woman: "You mean the ones sitting there on the coffee table?"
Man: "Where?"
Woman: "Right there in the middle of that table."
