"Schmarandieren" is a German slang term meaning "to be on the verge of circulatory collapse due to excessive drug abuse." It‘s the longer and more meaningful version of the word „Schmarandern“, which basically means the same.
The word is used, as one might first suspect, not to draw attention to a medical emergency, but either to tell one's circle of friends how fucking high one is or as a question to make sure that the person in question is equally unearthly high.
