When any villain or hero in a Hollywood movie repeatedly and/or unnecessarily cocks their gun for dramatic effect.

Ah, yes, nothing else says elite tactical training like a little dramatic hollycocking, because obviously that fully loaded gun needed one more unnecessary click to save the day.
