This term stems from the heroin use by famous bebop and jazz musicians such as Charlie Parker, Miles Davis, and John Coltrane. however, because jazz musicians historically transported marijuana around the US in the early 1900s, this term may be used to describe usage of many drugs.

The penis is inserted into the vagina all the way in using a slow, smooth stroke. You pull out slowly as well right until the head of the penis is about to fall out and then you sink back in slow.
