It is used by many people for/in different ways. Some will say it as they are attracted to thick thighs, some say it to feel better about their thick thighs, and rarely people will say it as they know research shows that those with thicker thighs are less likely to die from heart complications.
It was popularised by a number of groups. The main group is those who wear merch of anime girls with thick thighs. The merch often says "thick thighs save lives". The people that wear the merch also often say the slogan.
Those who have thick thighs and/or want to promote body positivity will often say the slogan and wear clothes with the slogan on it. Some guys will also say the slogan as they are attracted to thick thighs.
