It's unclear by what method this injury would occur, but presumably cardiac arrest or colloquial heartbreak. Also, may refer to the pain one feels in his/her gut upon seeing someone extremely attractive. May be described as a punch in the gut. Someone who is hurtin' em may elicit wistful sights of strangers as he/she walks by.

Get the Don't Hurt 'Em mug.The automatic response when somebody says "Jesus Christ!" in exasperation. It implies that the person thinks you are divine compared to their sorry ass. There is also an undercurrent of blatant self-promotion, in that it's fine for them to call you the Lord behind closed doors, but you don't want them embarrassing themselves in public. This usually pisses them off, but it stops them from whining.
