Alternate Definition:
A leisure lunch taken with other proffessionals for the purposes of buisness. Often with clients or coworkers. 3 martini's = 3 hours.

Modern equivlant is two martini lunch which means to have a short three martini lunch

Bob and Susan took their clients out for a three martini lunch to discuss future products releases.

cigarette lunch

when you do have time to eat lunch, you smoke a cigarette instead. the cigarette suppresses your hunger tiding you over to the next meal.

Q:Hey, what did you have for lunch today?

A: nothing, i had a cigarette lunch

I have no time in between classes on tuesday to eat lunch, so i have a cigarette lunch while i walk to my next class.

ladies who lunch

Rich middle-aged women with no jobs or other meaningful way to occupy their time, who descend on upscale cafes around lunchtime to hang out with each other more because they have nothing better to do than because they like the company. Generally despised by cafe workers.

I don't know why Ina Garten has this "ladies who lunch" persona -- you know she busts her ass when she goes to work.
