This usually happens by glitches and is not an intention feature of the game. A lot of times, you have to start from a different save point or even restart the game entirely to escape it.
Some examples of softlocking are: being unable to progress due to being stuck in an area without the tools to get out, a corrupted save file so that you cannot really progress on that save, etc.
