It means a woman who is a fan of gory things like horror movies, shock films and snuff films
This is the combination of two words, "Okumi" which is the Japanese kanji for "Gore" and "Joshi" which is the Japanese kanji for woman or lady which can be seen in the word "Fujoshi" (Rotten girl)
