A one in a million chance(or far less) of something happening. So ridiculous one might ask seriously? In most instances the circumstances are unheard of or improbable.

"vandertuuked" -dropping a full bottle of wine(cork down) on your foot and it going through your dress shoe tearing multiple tendons in your foot.
