What the hell just happened? 

A phrase used to show puzzlement over an occurrence. Oftentimes in the show South Park it is spoken by a character after they have seen a parody of something commonplace in our society. In such instances it often draws attention to how out of the ordinary accepted actions can be.

well, what happened was... 

used to preceed an exaggerated story about a mistake/problem that could (usually) be summed up with a short, more accurate, explanation.
"well, what happened was... a dog saw a squirrel and ran three times around the tree and by the time I finished helping the dog, that's when my sister's car was stolen by Iraqi Ninjas. So I chased them down and punched one in the face, he burst into flames and then I got back into the car, drove here and that's why i'm late for work."

That just happened

(Phrase) The only thing you can say when something so audacious happens that you have no response beyond iterating that the event occurred in the first place.
Person 1: I just got a tattoo of Justin Bieber on my ass

Person 2: I... Uhhhh... That just happened.

If something went completely downhill you’d just say a 2020 just happened

I'm telling you what I want you to think happened and taking my precious time so that it comes out believable.

What had happened was 

A term commonly used when you are about to speak total bullshit for the next few moments. The statement after the phrase may be true, but the statement is irrelevant and doesn't apply to the situation

Saying this phrase to someone such as a teacher or significant other, will get you in trouble because by now they know this phrase. Saying it around your friends will cause them to laugh if you are seriously about to lie. They know it, I know it, your mom, dad, uncle, dog, and second cousin twice removed all know you're going to lie.
***You are in the principles officer after sleeping late and you recall yesterday you had a flat tire***

Principle: Little Jimmy, why were you late again this morning.

Little Jimmy: Well... uh.. you see what had happened was... was that I had a flat tire and I could not make it to school on time because I cannot drive with a flat tire.
