it is used to describe someone, usually in a game, performing extreme acts of confusion and schizoprenic behavior while also being similar to that of an NPC behavior but still being very unsuccesful in their goals

person2: "gotchu" *proceeds to punch walls and looking at the ceiling*
person1: "bro wtf are you doing?? we about to lose the round!!"
person2: "i got this bro" *proceeds to die*
person1: "nahh bro that was some mad maro syndrome"

Can be used as a synonym for „shopping addiction“.
