catastrophic malfunction

In the gun world a catastrophic malfunction is characterized as a gun malfunction where it's destroyed/severely damaged usually from it exploding. Malfunctions like these easily can cause injury or even death to the user.
A catastrophic malfunction is not good considering a bunch of metal can be flying in your face.
