An annoyingly slow, blocking bottleneck in an otherwise efficient AI system, retained because current technology, law, or convention still requires human oversight, approval, or intervention.

Mira had already reviewed the logs, generated the plan, run the tests and prepared the update. Then everything stopped because the human in the room had gone to make coffee.
