A complete failure or blunder, especially involving anything related to the operational security of an organization— especially involving critical or senstive information that is compromised or mishandled due to carelessness or incompetence.

Origin: A blend of OpSec (operational security) and oops, coined as a contrastive term to copacetic, used to describe situations where security protocols are breached or mishandled in a spectacularly incompetent way, highlighting the failure to execute properly.
