Does not usually refer to any particular sexual act taking place as previously stated,but merely refers to someone being perceived as someone who masturbates,simply for purposes of an insult be it truth or complete fiction.
Can also be used with sexual reference although there are more suitable words,namely: handjob/wankjob
May also used light-heartedly between mates basically meaning dumbass
