Derogatory term for simultaneously - used to describe a completely effed up situation caused by careless activity with disastrous results - e.g. major upgrades that requires several following 'fixes' or upgrades due to buggy or sloppy coding.

Another term for similar effed situation - e.g. TARFU, FUBAR, BOHICA, totally effed-up, polis pos upgrade
