Its a medical fetish created in fivem GTA. Where you sign up for a appointment at the hospital with Dr. Mariella Chase. The hospital is undercover as a normal hospital, but is really a Whore House Hospital. The process starts off normal, you are anally probed, the doctor then proceeds to have a gander. Soon after she hooks the prober up to a Chainsaw like device and turns it on high. The only way to stop this process is either by passing out, or defecating your self.
