The bottom of the bottle is also burned/pierced to allow water to be let out when the lettuce is burned. With the water escaping the bottle, it is sucking the milk out of the weed into the bottle making a one shot one kill cloud.
Origin: The word Tłok is a direct translation of piston in polish. It reflects how the smoke is pushed into the bottle and then sucked out.
