a nut sack that is nine inches or someone who has recently lost a lot of weight and their excess skin resembles that of a nut sack

Joe: Wow have you seen Lisa lately?
Justin: Yeah i heard she lost a lot of weight.
Joe: Yup she looks pretty good except for the nine inch nut sacks hangin off her arms.
