An dumb-ass error that causes you and/or others a lot of mild-to-moderate "pain", either physical or mental/emotional.

Campus electrician: I forgot to hook the water-heater back up after I re-did the wiring in the utility-room, and so all da poor students hadda take cold showers the next morning and grumpily showed up for classes wif blueish skin and painful cramps --- what a mistache I made!
