This is a euphemism for the state in which someone is in need of a bowel movement, i.e. that feeling that you need to take a crap.

Man, I have been hauling lumber all day, I don't know how much longer I can hold it.

when a person falls inbetween the crack of the wall and bed or other area and someones else is pulling the person back onto the bed. This attempt is usally futile and ends up with the persons ass crack hanging out and half on the floor.
