It's derived from the term 24/7. 24/7 is actually a fraction, so we can calculate it in its repeating decimal form. 24/7=3.428571...

"3.4" is just used because it's much easier to type and say than the previously mentioned decimal equivalent.

3 am piss

A 3 AM piss is when someone is awoken at 3 AM by the urge to piss. The person will then typically get out of bed grumbling like a drunk fucker and walk into the bathroom where the lights will hinder his vision for about 3 minutes. Because of this short term blindness the dude will piss all over the seat as he exclaims fuck over and over again.

"Dude I had an awful 3 AM piss last night I couldn't see for a fucking week."
