Its the guilt you feel when you have schadenfreude, know you shouldn't logically, but still have it anyhow, even with the guilt.

Trump getting COVID seems to have caused a lot of schuldschadenfreude among people who still agree he's human so they feel guilty being happy for his suffering, but can't help thinking he got the karma coming to him so they can't help themselves.
