It could also refer to anyone that creates drama over something that's unnecessary, and trivial. Members of the gay community often refer to drama queens as "Karens".

It’s typically used by middle aged people to basically call out the very things they do themselves.
Since there’s no male name used as much as Karen it’s basically used to shut woman up just by saying “OK Karen” It’s also used by people who have nothing interesting to say to defend a point so they call someone Karen.
Like all things it’s ran it’s course but the boomers just can’t help calling other boomers Karen too feel cool.

"Karen" is middle-aged and up, has short blond hair, and always wants to speak to the manager about the inconsequential infractions of the younger service workers. She has also been known to engage in racist police calls (although in reality those women have all had other names) and has merged with "Becky." Calling out that this meme is itself sexist and tone deaf will also get you called Karen.

Get the Karenmug. The human version of the rare plague rat, these people are often observed as blonde, white, and female. Even during a worldwide pandemic, they refuse to wear a mask, which is how we can tell that they are closely related to the plague rat. They are commonly found in a store,(a Walmart is where you'll find them most) arguing with a more reasonable employee. They attack their prey not by killing them, like most animals, but potentially giving them a deadly virus by not practicing social distancing, not wearing a mask as previously said, and by aggressively demanding to speak with the employee's manager. Also note that their transportation is usually a Honda.
Employee: I'm very sorry ma'am but you are required to wear a mask, it's the law!
Karen: It's my right not to!
Employee: Ma'am I'm trying to be reasonable but you're making it diffi--
Karen: Don't you dare talk to me like that! I would like to see the manager!
