Its a derogatory term for woman. Its the short version of "sandwich maker."

Guy 1: "I got in an argument with my girl last night and I called her a name."
Guy 2: "Well, what did you call her?"
Guy 1: "I called her a smaker."
Guy 2: "OMFGz no way.... what did she do?"
Guy 1: "Made me a sandwich... duh."

I got "smaerd" missioning last weekend. The hos were sloppy.
