You cut a hole in some bread. Ideally pumpernickel and insert your John Hancock into said hole. Similar to "dick in a box" but with bread.

Girlfriend: "Hey, babe. Can you make me a sandwich."
Boyfriend: "Sure. You can eat my pumperdickle sandwich I'm making."
