First fail is when a person posting a comment on a website posts the word "first", but actually misses first place because someone else posts a more meaningful comment a split second before them.

First-class fail 

A first-class fail is a euphemistic term for failing a paper with the highest possible failure grade.

If you needed a C- to pass, for example, then receiving a D+ would count as a "first-class fail".
