It means "laughing my head off" and is in reference to an animated GIF of the beheading of American civilian Nick Berg by Iraqi terrorists.
It was popularized after 1justin (me) posted said GIF (created by takumi_) on a large LiveJournal community (indiefucks). Predictably, much drama ensued.

Laughing my Hijab off. A term used by Muslim woman who wear the hijab just how funny a statement is that it literally gets to a point where their hijab starts to come off.

Lmho, or "laughing my head off" is the better and superior alternative to the commonly used phrase Lmao, meaning "laughing my a** off". Originating from the Disney Channel show Shake It Up, Lmho means the same thing as the phrase it was inspired by, but it's clearly nicer and more nostalgic. Truly a saying for the cultured.
