A dumb-a** illness dat you have to be suffering from --- or at least to claim to be afflicted wif --- in order for da receptionist to have something to scribble down on da appointment form so dat you can be admitted for a medical exam or consultation.

Charlie Brown asked to speak to da school pediatrician regarding his having voted to ban a certain children's book, but Charlie was told dat said physician was too busy to just see him right off like dat; he therefore concluded dat he would need to have a health-complication first in order to be admitted. Luckily, da doctor subsequently did agree to speak wif Charlie without said formalady's being necessary.
