"I got the sugar" is a term used by ignorant ghetto dwellers. It refers to there diabetes and is usually a response given to a medical professional.
Doctor: Mr. Jones, besides being a drunk, do you have any medical conditions?
Mr. Jones: I got the sugar
Doctor: Do you mean diabetes?
Mr. Jones: Yes, and I gots the pressure too!
