The word lost all derogatory connotations about a century ago, except for ambiguously white kids with identity issues who decided to cling to an ethnicity they neither belong to or know anything about (see guido). These are usually the idiots who consider Italian to be a separate race.

a person of southern Italian and African ancestry living in the US. used to refer to the less ethnically diverse southern Italians who closely resemble their African American cousins by driving Cadillacs and wearing gold chains and having darker skin and other ethnic features not found among europeans.
