Originally refers to the 1982 Chicago Tylenol murders where cyanide was slipped into Tylenol bottles. Johnson & Johnson halted production and quickly, recalled all bottles. The move is likely what saved the Tylenol brand from this incident.

This expression refers to act of not doing what you are supposed to do and knowing that you will pay for it later.
