Refers to a blow job given by an elderly woman, usually a "cougar". The smoothie refers to the womans "smooth" gums and the texture of the man's semen . This term is regional, and is used commonly around the "Villages" in Central Florida.

I saw Steve getting a blue haired smoothie down by the gazebo. Wow, she must have been eighty something!
