Getting incredibly hammered and wandering into a local pizzeria. Upon drunkenly stumbling into said pizzeria, the subject walks into the restroom of the opposite gender, locks the door, sits on the floor, and vomits.

Very drunk. In British slang, "pissed" means drunk (not angry, like in the US), so pissed to the gills is like being so drunk, you're SWIMMING (hence, the GILLS) in alcohol.
