Almost certainly originates from a TV coffee advert of the 70s/80s starring Gareth Hunt. He holds several coffee beans in a half-clenched fist then shakes them in a way reminiscent of a hand job. The relationship with Gareth Hunt is further strengthened since he was a considered to be a bit of a wanker.

Grasp large knife. Plunge said knife into someone's chest. You have now completed the "Edmonton handshake". Known as "stabbing" or "knifing" in less crime-ridden cities. Most often used for no apparent reason, likely on someone elderly, smaller than you, or someone with nicer clothes than you. Be especially careful of the Edmonton handshake after denying an Indian a cigarette.

My friends are visiting from out of town and I want them to get the real Chicago experience so I'm buying them a round of Chicago Handshakes!
