means to pour a 4oz of promethazine in a drink to make drank (usually the drink is soda(mainly a 20oz sprite) aka dirty sprite)
I'm pretty sure the phrase comes from Texas namely Houston where a lot of rappers originate and often mention mxing lean in this way. don't do drugs

Kangaroo Pouch Surprise

A Kangaroo Pouch Surprise is when you clasp your penis in one hand while the other hand is in the shape of the middle finger gesture. The middle finger pushes down on the head of the penis, causing it to go inside the penile shaft, forming a baggy of skin.
