It's either you don't here something that somebody said correctly and you want them to repeat it or if you yell (or put in capital letters if you're are writing it) the "what" it means that the other person just told you something that you don't like or unpleasantly surprised you.

You: (busy preparing for the fair) I'm sorry, what?

you don't realize how many people care for you. even though you say that people do not care for you, don't die. don't cut yourself. yes, i've dealt with this before and i almost did die, but i just realized that it's you who cares for you, dont kill yourself. people are there who love you, even though im a stranger i love you so much. please do talk to someone if youre having suicidial thoughts.

I'm tired

A short phrase (with variations such as "i'm pretty tired") used as a simple explanation when a person doesn't want to do something but won't tell you the actual reason as to why - usually used as a replacement for "i can't be bothered" or "i don't want to". Also used as a form of rejection without openly saying it.
A: Hey man, you still coming to the pub tonight?
B: Nah.... i'm feeling pretty tired now.
A: You woman...

Guy: Hey.. d'you want to go out for dinner tonight?
Girl: Um.. nah.. I'm tired.. sorry
Guy: Oh, well, how about tomorrow?
Girl: No.. I.. have to wash my hair...
